Thats the ladder thats welded on to the 200 foot spire. I took the top picture from the top, But I realised I hadn't taken one of myself by the time I had clambered down a bit. Hence why you can see rungs going up. Still hanging on by a couple of fingers whilst taking a pic (and whilst crapping myself) was no mean featpauly p said:

Just in case anyone else wants to do it. Here's another pic to describe it.
Get the elevator to the 86th floor. Go up another floor to either Petronas presidents office or the Malaysian PMs office depending which tower you are in. Up another floor to the cleaning gantry, as you can see here.
Climb up some more internal steps to the just above the globe structure. Open the door and climb the rungs a couple of hundred feet
PS don't look down
